Built-in penis

The best definition for a built-in penis is a normal-sized penis, but with inadequate exposure of the phallus (penis).

It is a genetic anomaly where the deep abdominal fascia (tissue located in the suprapubic region) is not well fixed to the pubis, causing the suprapubic tissue to project over the penis. Furthermore, the scrotal dartos (tissue located beneath the skin of the scrotum) is also not attached to the base of the penis. This inadequate attachment of the tissues, both on the dorsal (top) and ventral (bottom) sides of the penis, gives the impression of a phallus that is smaller than it actually is.

A very common mistake in pediatric consultations is to advise parents that, as the child grows and develops and consequently the penis, the phallus will naturally become more exposed, but this will never happen without proper treatment. A child with an embedded penis will be a teenager, an adult and an elderly person with an embedded penis, suffering bullying and frustrations throughout their life.

Furthermore, over time, weight gain and the effect of gravity on the body cause the penis to become increasingly “hidden”, causing men to mistakenly believe that their genitals are shrinking even further.

Diets and physical exercises to reduce weight can improve the exposure of the phallus, but, as the problem is genetic in nature, only surgical procedures can definitively correct the defect.

The surgeries indicated to correct this defect are suprapubic dermolipectomy associated with genital lifting, in addition to procedures to fix the scrotum to the base of the penis. In selected cases, small suprapubic liposuctions can help. The results are generally satisfactory, with a significant improvement in male self-esteem.
